I have just upgraded my ISAPI system. One thing I am not sure about is CacheDir property in Servercontroller. I have currently let it blank, and the system runs. But is it best to specify a folder here - and if so where is it best for that folder to be?

It is a good idea, but not necessary. Usually, I create something like:

C:\Temp\MyIWApp

If I have more than 1 IW application, I create multiple directories, below C:\Temp, i.e.:

C:\Temp\MyIWApp1
C:\Temp\MyIWApp2
